Happy-path screens do not show whether a website helps visitors recover. An error state inventory makes failures visible during design and gives reviewers a shared set of conditions to test before acceptance.

List failures by user task

For each form, search, account action, integration, and content request, record the trigger, message, visible location, retained input, recovery action, support route, logging behavior, and owner. Separate field validation from service failures and permission problems.

Write useful messages

  • Explain what happened in plain language.
  • Place guidance near the affected control.
  • Preserve valid input whenever safe.
  • Offer a specific next action.
  • Avoid exposing technical or sensitive details.
  • Design beyond the message

    Specify focus movement, announcement to assistive technology, loading transitions, retry behavior, and what happens after repeated failure. Include narrow screens, zoom, keyboard use, and slow responses in the review.

    Test controlled failures

    Use safe test environments or supported simulation methods. Record the input, build, expected state, actual state, and recovery result. Coordinate security-sensitive failures with the appropriate owner rather than attempting disruptive tests on production.
